A high cost benefit

Benefit: Pays claims for medical expenses covered by a certificate issued by an insurance company for any off the job injury or sickness.

Rates are determined by: Industry, age, sex, plan design and severity and number of preexisting conditions in the group.

Cost per person: Cost can very from $65.00 to $450.00 per employee with non-composite rates. Composite rates, which are an Average cost, is $185.00 per employee.

When a group is larger than 15 average or composite rates are recommended.

How much is customary: A one or two million dollar maximum, with a $500 to $1,000 dollar deductible, 80/20 cost share to 10,000 with an office visit co-pay, and a prescription card is the average plan of choice.

Who should pay: Employer is required by most insurance companies to pay a minimum of 50% of the employee's cost. Employers may pay as much as 100% of employee and dependent costs. Groups of 2 to 50 are guaranteed issue, but the good health premium can be increased to as much as 67% because of preexisting conditions in the State of Texas.
