A Low-Cost Benefit

Benefit: Pays claims for dental expenses covered by a certificate issued by an insurance company.

Rates are determined by: Industry and plan design. 

Cost per person: cost can very from $18.00 to $35.00 per employee. Average, is $23.00 per employee.

How much is customary: $1,000 or $2,000 maximum benefit per person per calendar year. Preventative at 100%, $50 Dollar deductible for minor and major, minor at 80/20 cost share, and major at 50/50 cost-share. (Orthodontics optional)

Who should pay: Employer is required by most insurance companies to pay a minimum of 50% of the employee's cost. Employers can pay 100% of employee and dependent costs. Participation requirements are 75% for eligible employees and 75% of dependents.
